Campaign Disclosure Statements

Pursuant to the Political Reform Act, candidates and campaign committees are required to file campaign disclosure statements which provide detailed accounting information on money spent or contributions received in support or opposition of a candidate, campaign, or measure. Elected officials and candidates may be required to submit one or more of these forms, depending on their campaign finance participation:

  • FORM 460 - Recipient Committee Campaign Statement (Government Code Sections 84200-84216.5)
  • FORM 450 – Recipient Committee Campaign Statement – Short Form (Government Code Sections 84200-84216.5)
  • FORM 470 – Officeholder and Campaign Statement – Short Form (Government Code Sections 84200-84216.5)

Additional Official Report Forms

  • FORM 801 – Payment to Agency Report: Certain payments received by state and local government agencies, including payment for an official’s travel expenses for the purpose of facilitating the public’s business in lieu of a payment using agency funds, and a payment that would otherwise be considered a gift or income to the benefiting official, but is instead accepted on behalf of the agency.
  • FORM 806 – Agency Report of Public Official Appointments: Additional compensation that officials receive when appointing themselves to positions on committees, boards, or commissions of a public agency, special district, or joint powers agency or authority (FPPC Regulation 18705.5).
